Getting Medical Document

To receive a recommendation for medical cannabis you must be assessed by a licensed healthcare practitioner. Once assessed, the practitioner will determine if you are a good candidate for medical cannabis treatment. Once approved, they will issue a medical document and you’ll be good to go!

Yes.

Typically, your first medical document will be active for approximately 3-6 months.  This allows you to test the treatment and ensure it’s working for you. After that, you are required to renew your document so that your healthcare practitioner can evaluate your treatment.  If the treatment is working, your medical document will typically be renewed for 6 months to 1 year.

If you start the process but realize you don’t have time to finish or complete the consultation, you can return when is convenient for you. Practitioners are available 7 days a week.

With your consent, your medical document will be forwarded to Syndicate for your convenience. Should you require a copy, please connect with Syndicate Client Care Team.

Yes.

You can contact Syndicate at any time to transfer your document to another licensed producer. 

Renewals

To renew your medical document, book an appointment online through the Appointments tab in your Account Profile. If you need help along the way, please call our Client Care Team at 1-888-733-7963

Once we successfully receive your Medical Document and any other paperwork that is required, you will be able to place your order within 1-2 business days. You will also receive email confirmation from Syndicate once you are ready to reorder.

Syndicate sends out renewal communications to clients via email.
