To become a registered client with Syndicate, we require a fully completed Medical Document from a registered Health Care Practitioner. You will also be required to fill out a Registration Application, which can be completed online, or printed from our website and sent back to Syndicate via Canada Post.
If you already have a medical document, it can be split between a different licensed provider, or transferred fully to Syndicate.

Active Starseed Patients: To register for Syndicate, simply enter your email and create a new password. Your completed account will be fast-tracked to Syndicate.
Yes. Upon registering online with Syndicate, we will guide you through booking a free consultation with a health care practitioner at one of our clinic partners – HelloMD or North Star Wellness.

Yes. Syndicate will accept other medical documents if the mandatory information is provided, and that it meets the same criteria as the Syndicate Medical Document.
A Responsible Individual or Caregiver is an individual who is responsible for the client of Syndicate. The Responsible Individual is required to complete and sign the Registration Application Appendix A1 for Syndicate on behalf of the applying patient.
